Complete the New Military Student Intake Form, follow instructions below based on your program pathway.
**On this form, please upload the applicable required documentation: DD214, Military Leave and Earnings Statement (LES), Military Orders. If utilizing VA educational benefits, please upload your Certificate of Eligibility or Statement of Benefits.**
Non-Degree Program Students (Course & Certificate Programs)
Complete this form prior to registering for courses. Completing the form prior to registration will ensure your 30% military tuition discount is applied to your account.
Degree Program Students
Complete this form after receiving your notice of acceptance to Berklee Online. Completing this form after notice of acceptance will prevent any delay in processing your 30% military tuition discount.Go to the Military Intake Form
Step 2 (VA Education Benefits Students Only)
Students utilizing VA education benefits should complete the VA Certification Request Form.

Complete this form EVERY SEMESTER after you have finalized your registration. If you make changes to your registration or schedule after submitting this form, please email firstname.lastname@example.org.Fill Out the VA Certification Request Form
Note: This form is for students utilizing VA education benefits (Post 911 GI Bill, Veteran Readiness and Employment VR&E, Chapter 35, Chapter 30, Chapter 1606, etc.). This form is NOT required for students eligible for Military Tuition Assistance.
